As you may know, organizations using CIHQ accreditation for deemed-status purposes must inform patients, or their surrogate decision-maker, of the right to file complaints regarding quality of care concerns or safety issues to CIHQ. This information must be posted on the organization’s website (if there is one), and in registration areas at all of the organizations sites of care (both inpatient and outpatient). The required information is in the PDF attached to this email. We also have the required information down below in red, in case the PDF does not work.

In reviewing your organization’s website, we are unable to locate this information. If the below information is currently on your website, please provide the location or a screen shot of the web page.

If this information is not currently on your website, please have this information posted within the next 30 days.
